Graduate of an accredited university with a B.S. or Doctorate in Occupational Therapy that the AOTA recognizes.
OT holds a current license and/or registration as an Occupational Therapist in-state as applicable.

Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
