The Registered Nurse Case Manager position is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community. Valid nursing license in the state of NH
~2+ years of experience in skilled care preferred
~1+ year of experience in nursing supervision preferred
