Job Description
¨Home Care Client Coordinator oversees services provided to individuals in their own homes and communities, who need assistance caring for themselves as a result of old age, sickness, disability and/or other inflictions. Home care may include assistance with the activities of daily living, housecleaning, laundry, meal preparation, transportation, companionship and respite.
¨ Client Coordinator ensures that employees deliver services in a caring and respectful manner, in accordance with relevant Agency policies and industry standards.

¨ Reports to Director
Responsibilities/Activities
¨ Ensure Agency policies and procedures and industry standards and regulations are followed.
¨ Assist with the coordination, evaluation and planning of home care services.
¨ Assist the Director with administrative activities and optimal use of resources.
¨ Develop, implement and evaluate strategic plans, goals and objectives.
¨ Coordinate staffing activities.
¨ Supervise work activities of designated employees.
¨ Develop and instruct employees in the use of practices, procedures and equipment.
¨ Prepare budget reports based on compiled financial and statistical records.
¨ Provide leadership to employees.
¨ Liaise with community resources and other agencies.
¨ Any additional duties necessary for seamless continuity of services
Required Knowledge
¨ Knowledge of standards of practice and scope of practice, in accordance with state regulations
¨ Comprehensive knowledge of other health care disciplines and their role in consumer and family care.
¨ Knowledge of personal care and home management skills.
¨ Knowledge of principles and processes for providing consumer and personal care services, including needs determinants, meeting quality standards and evaluation of consumer satisfaction.
Job Type: Full-time
Pay: $20.00 - $23.00 per hour
