CRC is a not-for-profit community based organization which exists to establish, maintain and operate Intermediate Care Facilities, Individual Residential Alternatives, and to provide programs of care, service, habilitation, rehabilitation, and social/recreational activities geared to the functional capacity and developmental needs of each consumer in our care, so that they may be helped to experience living independently, making meaningful choices, enjoy personal relationships, communicate freely and receive effective services.
SUMMARY
Under general supervision of the Director of QA, is responsible for conducting agency investigations and to provide administrative support to the QA department and agency operations as it pertains to investigation follow up (CAPS).
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Investigator will include, but are not limited to the following:
- Conducting agency investigations within regulatory timeframes as assigned.
- Provide administrative support to the Quality Assurance Department and agency operations.
- Provides training to staff regarding conducting investigations, incident reporting, and related topics.
- Assist with management and tracking of information in OPWDD’s IRMA database and interface with the Justice Center to ensure incident closure.
- Coordinates completion of annual OPWDD Utilization Reviews for Residential Programs.
- Assists in completion of required Justice Center target letters, submission of SCR target searches, and submission forms for Investigative Records for Abuse and Neglect Allegations.
- Interfaces with OPWDD incident coordinators to ensure proper classification of incidents.
- Assists in preparation of annual trend information for incidents.
- Performs other administrative duties as assigned.
QUALIFICATIONS

- Fully conversant with OPWDD and agency selected computer software programs.
- Excellent knowledge of OPWDD regulations/incident management regulations.
- Must be a certified investigator or be able to pass the certification course within 60 days of employment.
- Ability to read, write, speak, and understand English fluently.
- Ability to work independently and manage multiple tasks simultaneously.
- Ability to prioritize routine work without continuous oversight from the Director of QA.
- Excellent interpersonal skills are required to interact effectively with coworkers, employees, and other departments.
- Due to the sensitive nature of the position, must be able to maintain the strictest of confidentiality.
- Excellent organizational and critical thinking skills.
- Excellent verbal and written communication skills.
- Excellent presentation and teaching skills.
- Frequent traveling to and between program locations.
- May require evening and weekend work.
- A valid New York State Driver’s License and must maintain it in good standing is preferred but not required.
EDUCATION and/or EXPERIENCE
- Bachelor’s Degree in a Human Services related field.
- At least 3 years of prior work related experience conducting investigations and program supervision.
LANGUAGE SKILLS
Ability to read, write, speak and understand English. Ability to read, analyze, and interpret reports and documents. Ability to respond to common inquiries. Excellent written and verbal communication skills required.
MATHEMATICAL SKILLS
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Basic understanding of statistics.
REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables simultaneously. Ability to resolve conflicts and mediate stressful situations. Ability to demonstrate critical thinking skills with logical outcomes, based upon collected evidence and recommendations.
