Qualifications
Must be a Licensed Social Worker (LSW) in the State of Ohio with a minimum of a Bachelor's Degree in social work. Experience in long term care and/or the geriatric population with an emphasis in the areas of admissions and counseling. Excellent written and verbal communication skills.
Essential Functions
A. Medical Social Service Needs
Oversee the provision of medically related social service needs. (i.e., to assist the resident in maintaining or improving the ability to manage everyday physical, mental, and psychological needs) Ability to:
1. Assess the resident and identify and seek ways to support the resident's individual needs and preferences, customary routines, concerns and choices. Assist the resident and his or her family or responsible party to adjustto life at Jennings Hall.
2. Maintain contact with family members (with resident's permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning.
3. Assist with financial and legal matters, e.g. applying for pensions and other benefits, Medicaid and Medicare, advance directives, referrals to lawyers and referrals to funeral homes for preplanning arrangements.Coordinate financial arrangements with the Homes Business Office.
4. Act as a liaison with social, health and community agencies and be responsible for referrals of residents when appropriate. Ensure documentation in the resident's record concerning the results of such referrals.
5. Assist the resident and/or staff in obtaining needed adaptive equipment,clothing and personal items

Coordinate family and community resources as needed to solve financialneeds and to promote emotional security.

Coordinate and participate in family and resident activities designed topromote social interaction, reality orientation, and intellectualstimulation.

Assist in the organization or community events related to residentsneeds and interests.

Plan, schedule and implement room visits for residents who do notleave their rooms.
B. Care Plans
1. Assess the resident and complete assigned portion of the MDS + and theSocial Services Assessment within 14 days of admission. Complete quarterly hospital returns and significant change MDS per facility.
2. Document the social services care component of the Comprehensive Care Plan for each resident as identified by the interdisciplinary team within 7 days of completion of the MDS.
3. Review the resident's status and progress at least quarterly and when needed and document in a social services progress note.
4. Be responsible for completion of a discharge plan in accordance with facility discharge policy.
5.Coordinate assessments and care with consultant psychiatrist and consultant psychologist.
6. Attend weekly Interdisciplinary Care Plan Conferences and functionas an integral part of the care plan team.
C. Admissions
1. Answer intake calls, arrange intake/interview visits and tours, and facilitate the admission process with applicant and or family. Coordinate admission with nursing and other departments.
2. Maintain a waiting list of applicants that is current and up to date with each individuals needs and projected neighborhood.
3.Attend admissions committee meetings; participate in decision making or room changes within the facility that are appropriate to meet the residents' needs; facilitate admission procedure for resident next on the waiting list.
4. Coordinate hospital returns with nursing department.
5. Perform pre-admission evaluation visits to assess applicantsin their own homes, hospital or other nursing facility; (assessesresidents in hospital prior to discharge back to facility).
6. Facilitate the day of admission process; meets and greets resident and family, orients to the room and Home, reviewsand obtains signatures on all admissions forms.
D.Discharges
1. Assist staff and family with proper disposal of resident's personal property following death.
2. Attend funeral services when appropriate.
3. Assess resident's ability to return home or to another appropriate environment. Coordinate communications and documentation with community agencies or receiving facilities, including the completion of a discharge/transfer summary.
E.Counseling
1. Develop one-on-one relationships with residents and families as needed for counseling.
2. Provide or arrange provision of needed counseling services about death or dying and other emotional, mental, environmental or physical limitations.
3. Lead resident and family support groups.
4. Coordinate resident's religious and spiritual needs with Director of Pastoral Care or resident's community pastor.
F.Residents Rights
1. Participate in-service education and attend all in-service sessions relating to Social Service responsibilities.
2. Assist in building relationships between residents and staff and teaching staff how to understand and support residents' individual needs.
3. Assist in promoting actions by staff that maintain or enhance each resident's dignity in full recognition of each resident's individuality.
4. Counsel and advise residents and surrogates regarding advancedirectives. Assist in preparation of appropriate documents. Assist to determine how they would like to make decisions about their health care, and whether or not they would like anyone else to be involved in those decisions.
5.Explain resident's rights to residents and residents' representatives; ensure the implementation of resident's rights and act as the resident's advocatewhen appropriate. Facilitate the use of the Grievance procedure whenappropriate.
6. Coordinate the distribution of residents mail to responsible parties.
F. Other
1. Complete all documentation in a timely manner according to facility policy and procedure. Utilize computer MDS and Care planning system and other programs as appropriate.
2. Participate in the theft loss program by interviewing residentsappropriate and assisting to maintain clothing and property list
3. Coordinate with Life Enrichment Department to meet socialization needs of the resident.
4. Be responsible for the safety of residents under his/her supervision.
5. Observe all facility safety policies and procedures
6. Observe infection control procedures.
7. Accept assigned duties in a cooperative manner.
8. Assume accountability for data contained in the Staff Handbook.
9. Conduct appropriate and useful QA studies
10. Participate in Ethics Committee.
11. Participate in the review and development of policies and procedures for Social Service Department
12. Work flexible hours in order to meet the needs of the residents and families.
13. Work independently and to function as an effective team member.
14. Come to work as scheduled and consistently demonstrate dependability and punctuality.
15. Ability to understand and comply with the Corporate Compliance Program.

17. Perform other duties as assigned by the Director of Social Services
