Job Overview:
We are seeking a skilled and dedicated Pharmacist to join our team. Duties:
- Dispense medications accurately and efficiently
- Review medication orders for appropriateness and potential drug interactions
- Collaborate with healthcare professionals to develop patient-specific treatment plans
- Provide drug information and counseling to patients and healthcare providers
- Monitor patient responses to medication therapy and adjust treatment plans as necessary
- Conduct medication reconciliation to ensure accurate medication histories
- Participate in utilization review activities to optimize medication use
- Collaborate with the healthcare team to develop and implement protocols for pain management, pediatrics, acute care, critical care, and ICU settings
- Ensure compliance with all relevant laws, regulations, and standards

- Proficiency in medication administration and dosage calculations
- Excellent communication skills, both verbal and written

Requirements:
- Bachelor's or Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy
- Current pharmacist license in the state of New York
- Experience in r clinical pharmacy setting (preferred)
